Legacy Portal: ezAccess

Important updates about ezAccess:

  • As of Saturday, November 1, the ezAccess mobile app is no longer active and the web version has transitioned to view-only mode. This means you can still see your prior records and message history online. Please note this information will not be moved over to MyNorthwell.

  • Starting Friday, October 31, the ezAccess mobile app will no longer be accessible as we transition to MyNorthwell. Please use the ezAccess website version to review chart updates and provider messages.

  • Starting Tuesday, October 28, the ability to send or reply to messages with your providers in ezAccess will temporarily be paused until Northwell’s new digital system fully launches in early November. Until then, you will continue to receive chart updates and provider messages and can call your doctor’s office directly with any questions.

  • Once MyNorthwell is fully launched, you will have full access to communicate with your provider(s) along with additional features, such as managing visits, accessing test results and more.

Our teams are working hard behind the scenes to fine-tune your experience. We appreciate your patience during this exciting transformation.

Concorde Medical Group, Member of Northwell Health Physician Partners, makes your records available through our patient portal vendor, ezAccess.

ezAccess lets you and your authorized representatives review test results, medications, allergies, conditions, vital signs and other clinical information. You can also access information about upcoming appointments.
